What is the hospice Care Plan?


 The Care Plan is an individualized plan of care that is developed for each patient and their family. The hospice interdisciplinary group schedules meetings with the patient and family to develop a care plan to care for the patient’s needs, and assesses if the patient needs emotional or spiritual support during this difficult time. The Care plan is there to give direction to all the disciplines of Mission Hospice on how to best treat the patient with recommendations ranging from what medications to offer to if the patient needs emotional support from our Chaplains and Social Workers.
