RN – Case Manager

Location: Long Beach/ South Bay

Full-Time

We are currently seeking an RN (Registered Nurse) Case Manager to administer nursing care according to the plan of care to terminally ill patients. As a privileged member of Mission Hospice and Palliative Care, you will provide dignified and compassionate care to patients and families experiencing a life-limiting illness during their time of transitions. It is our mission to make a difference in the lives of our patients and their families.

Requirements:

Licensed Registered Nurse, in good standing in the State of California required.
Hospice experience preferred
Must possess excellent interpersonal skills particularly with family cultural/sensitivity
Current CPR certification
Valid driver’s license and current automobile insurance
Reliable transportation


Duties and Responsibilities:

Case Managers work collaboratively with Physicians, Medical Directors, Patient Care Coordinator, Social Worker, Chaplain and Certified Nursing Assistants as an integral member of the interdisciplinary team. Case Managers assume the overall responsibility for organizing patient care, develop the interdisciplinary plan of care, provide comprehensive nursing assessments, teach appropriate palliative interventions to family members, assess and implement a pain/symptom management program to enhance quality of life and provide the highest level of hospice care to our patients.

LVN – Team LVN

Location: San Gabriel Valley

Full-Time

We are currently seeking an LVN (Licensed Vocational Nurse) to provide nursing care according to the plan of care to terminally ill patients. As a privileged member of Mission Hospice and Palliative Care nursing team, you will provide dignified and compassionate care to patients and families experiencing a life-limiting illness during their time of transitions. LVN – LIAISON

Location: Varied Locations

Part-Time,Weekends

This is a part time LVN Liaison position throughout our coverage area.

Hours: 2 weekends a month, 9:00 am - 6:00 pm

Functions & Responsibilities:

- Responds to inquiries and requests for hospice information in a timely manner

- Explains the hospice program to patients, family members, and other referral sources in an accurate, complete and thorough manner, EOB's.

- Provides accurate and timely documentation of all consents to the Admission department.

- Demonstrates ability to manage several responsibilities simultaneously and meets productive standards of the organization

- Assignments are completed meeting standards such as accuracy, neatness, and thoroughness.

- Demonstrates dependability by adhering to time frames, punctuality and attendance policies

- Adapts to changing situations well and handles difficult situations in a tactful manner.

- Provides direct patient care under the direction of Team Manager and Director of Patient Care Services.

Qualifications:

*Currently licensed as a LVN in California

*Willingness to work in patients home in various conditions; ability to work flexible schedule and adapt to a change of assignment

* Valid Driver's License and Automotive Insurance required

* Valid CPR certification (American Heart Association)

* Valid IV certification

Education:

Completion of course of study as required acquiring LVN license in the State of California. Graduate of an approved school of Practical/Vocational nursing required.

IV Cert required


LVN – Visiting

Location: Varied Locations

Part-Time,Weekends

This is a part time LVN position throughout our coverage area.

Hours: Weekends a month, 08:00 am to 5:00 pm; 4 pm to Midnight

Functions & Responsibilities:

- Responds to inquiries and requests for hospice information in a timely manner

CHHA

Location: Pasadena, San Fernando Valley

Full -Time

Hours: Hours vary depending on location of patients.

Functions & Responsibilities:

Under the supervision of the Registered Nurse, Case Manager:

  • Assists patients with personal hygiene such as skin, mouth, hair care and bathing.

  • Assists patients in and out of bed and assists with ambulation.

  • Assists with prescribed exercises which patients and aides have been taught by appropriate health personnel.

  • Prepares meals, including therapeutic diets, and assists patients with eating.

  • Assists patients to the bathroom or in using commodes, bedpans or urinals.

  • Performs household services which will facilitate the patient’s self-care at home and are necessary to prevent or postpone institutionalization.

  • Assists patients with medications which are ordinarily self-administered. The home health aide shall not administer medications of any kind.

  • Reports changes in the patient’s condition and needs to the supervising nurse.

  • Completes records regarding services performed.

Qualifications:

*Current CNA and CHHA license.

*Willingness to work in patients home in various conditions; ability to work flexible schedule and adapt to a change of assignment.

* Valid Driver's License and Automobile Insurance required.

* Valid CPR certification (American Heart Association).

Education:

Completion of a California Department of Health approved training program as outlined in Section 74747 of Title 22 or its equivalent
