We are currently seeking an RN (Registered Nurse) Case Manager to administer nursing care according to the plan of care to terminally ill patients.As a privileged member of Mission Hospice and Palliative Care, you will provide dignified and compassionate care to patients and families experiencing a life-limiting illness during their time of transitions. Hospice experience preferred
Must possess excellent interpersonal skills particularly with family cultural/sensitivity
Current CPR certification
Valid driver’s license and current automobile insurance
Reliable transportation
Duties and Responsibilities:Case Managers work collaboratively with Physicians, Medical Directors, Patient Care Coordinator, Social Worker, Chaplain and Certified Nursing Assistants as an integral member of the interdisciplinary team. Case Managers assume the overall responsibility for organizing patient care, develop the interdisciplinary plan of care, provide comprehensive nursing assessments, teach appropriate palliative interventions to family members, assess and implement a pain/symptom management program to enhance quality of life and provide the highest level of hospice care to our patients.Benefits:

The Hospice Liaison represents Mission Hospice in activities involving professional contact with patients/families, physicians, hospitals/facilities, senior living communities, professional associations, and similar health groups and institutions, to inform them of Agency’s services. The Hospice Liaison will be responsible for representing Mission Hospice at conferences and developing personal relationships with referral sources. The Hospice Liaison will be responsible for meeting and/or exceeding goals as set by the Hospice Marketing Manager with approval from Senior Management.


  1. Must be a graduate of an accredited college or university or be a licensed professional.
  2. Must have excellent communication skills, the ability to interact well with a great diversity of individuals, and the ability to organize and execute the selling processes.
  3. Self-starter who is willing to learn and develop business contacts.
  4. Additional language skills an advantage (Spanish, Cantonese, Mandarin, Farsi, Armenian or Russian)
  5. Hospice or Home Health experience is a plus.
  6. Driver license and reliable means of transportation.

Nurse Practitioner


We are currently seeking a part-time or Per Diem Nurse Practitioner for our Inland Empire location. NP will be responsible for performing Face to Face assessments for Hospice patients under the direction of the physician. The NP will perform physical examinations and assessments of the patients for appropriateness of hospice care eligibility criteria for admission and re-certification of hospice care. Assesses patients clinical problems and health care needs. Prescribes or recommends drugs or other forms of treatments patient may need. Records physical findings, formulates patient plan and goals of individual patients for periodic review and evaluation by physician. May refer patients to physician for consultation. Discusses case with physician and other health professionals to develop a comprehensive patient care plan. There is potential for full-time employment.


Job Responsibilities include:

1. Coordinates schedule of patient face-to-face encounters with physician
2. Discusses case with physician and other interdisciplinary team members.
3. Prescribes or recommends drugs or other forms of treatment based on assessment of patient.
4. Completes required patient documentation for patient visits.
5. Attends IDT/Case conferences as needed.
6. Adheres to all organizational policies and procedures.
7, Demonstrates adherence to nursing profession code of ethics.
8. Promotes mission, philosophy and goals of agency with staff and community.

Current State license as a Nurse Practitioner
Valid driver’s license and Automobile Insurance
Ability to work as a team member
Minimum of two years experience in hospice, home health, clinic or health facility.
Excellent observational, clinical, written and verbal communication skills
